Neurology is a most interesting topic in medicine. It can be divided into two parts, the cerebrum and cerebellum, and then everything else. The cerebellum is an old artifact that controls many archaic functions whereas the human cerebrum is the central element of what makes us human. At least we think it does.I had the good fortune to re-learn much neurology from Marty Samuels a colleague and co-author of Dr Ropper at The Brigham and Harvard. Samuels is a brilliant instructor because of how he gets across the main clinical points. First, neurology is not like internal medicine. In internal medicine you consider all the things that can go wrong and then via a differential diagnostic approach attempt to focus on the problem. In neurology as Samuels note you find out where the problem is in the circuitry. Second, is Samuels discussion on "dizziness". This is the George Carlin approach to clinical teaching. Samuels notes that you must listen to the patient, and never put ideas in their head by saying, "dizzy like the room spins" for in so doing one goes down the rabbit hole.Robberts in this book give vignettes, tales of people who for the most part have cerebral problems. Not simple tumors which we can see on most current imaging, but complex presentations. That part is great. But then what he seems to miss in my opinion is the process of following the circuit, finding where the problem is, and then trying to ameliorate the problem.There are dozens of stories of patients and their problems, but perhaps having more clinical experience than most, but clearly not being a neurologist, I would have greatly appreciated the follow on. What was the problem and hod did that cause the symptoms and what could then be done to help the patient? All left unanswered.
